Please read this notice carefully.
This privacy notice explains how DELPHI CARD® Health Care Management stores, disseminates, and utilizes group health benefit and census enrollment information obtained from you about your company and its employees in response to your request for quote.
You may request that we withhold any information from insurance carriers or other entities from which we solicit quotes for your medical, dental, and or life insurance coverage.
Information You Provide to DELPHI CARD®
In order to complete the quoting process, DELPHI CARD® utilizes current in-force benefit designs allowing for the presentation of a quote suitable for comparison, including premium payment history. We may contact your company directly to obtain additional information, as necessary, to complete the process, or to answer any questions we have about your current plan's coverage.
You must also supply information about your enrollment numbers and those employees participating in the plan, including dates of birth, coverage types, and gender.
We may seek additional information from the above named sources as well as from your current plan administrator, broker, etc.
We may also, from time to time, need additional health information about physical or mental health conditions, health care provided, or payments for health care. The information we need may include health care providers utilized, medical services rendered, charges for those services, and medical diagnosis.
Only after obtaining all necessary release/permission forms and only after following all legal stipulations, will DELPHI CARD® attempt to obtain this information. And in no event will DELPHI CARD® share personally identifiable information such as social security numbers with third parties.
How Your Information Is Used
Benefit and Census Information provided as part of the quoting process will be shared with insurance companies and other third parties, only as needed and only as permitted by law.
For example, we may provide your Benefit and Census Information to the following types of third parties: insurance companies; disease management coordinators; and other firms involved in assisting in the current or future administration of your plan.
We may disclose Benefit and Health Information, as permitted by law, to companies that provide health care coverage or reinsurance services or administer health benefits for self funded plans.
Safeguarding Your Personal and Health information
We restrict access to your company Benefit and Health Information to those employees who need to know that information to provide services to you. We maintain physical, electronic, and procedural safeguards that comply with federal regulations to guard your Personal and Health Information.
We do not sell, rent, loan, trade, or lease any data, personal or otherwise, to any third party.
If you send us an email with a question, or comment, or data concerning your company’s request for quote, we will use your email address only to respond to you. We will store your email, email address, and a copy of our response for archival purposes only.
DELPHI CARD® reserves the right to change this privacy statement from time to time. This privacy policy is not intended to and does not imply any contractual or other legal rights in respect to DELPHI CARD® or any other party.
